Mortality Surveillance System (MSS) is one of the surveillance systems to track disease pattern changes and the basis for guiding effective preventive interventions. A cross-sectional study was conducted, combining qualitative and quantitative, to describe situation and difculities for mortality surveillance activities in commune health centers, Nho Quan district, Ninh Binh province and Thach That district, Ha Noi City in 2017. The results showed that CHSs have recorded and recorded information
on deaths in the community according to the A6 / YTCS book. However, these activities are not uniform and mortality information is mainly collected from village health workers (VHWs). There are 21/50 CHSs who do not have a fulltime staff member for teacher education; 22/50 CHSs do not have trained staff. CHSs still face many difculties in teachers’ practice because of the lack of tools to investigate. The technical guidelines for diagnosing the cause of death and coded according to ICD10 should be developed and used as materials in training courses to improve commune health workers’ and village health workers’ skills.
