Effectivenes of intervention to support applying family medicine principles at commune health station for hypertation management, 2016 to 2018

A controlled intervention study in hypertensive patients (hypertension) over 18 years of age to evaluate effectivenes of intervention to support applying family medicine principles at commune health station for hypertation management. The effectiveness of hypertension management was conducted in 1,719 patients at pre - intervention and 1,741 patients at post - intervention in 6 communes of Hai Phong city. The intervention to support applying family medicine principles at commune health station. The results showed that commune health stations appling family medicine principles produced better outcomes for hypertensive patients: reduce salt intake by 5.2%; increase physical activity by 16.4%; and 14.8% patients diagnosed higher than pre-intervention. The number of patients who were treated and adhered to treatment increased by 17.1% and 28.1% respectively. Before intervention, 13.8% of patients achieved target blood pressure, after intervention this index was 33.7%.
