During diphtheria (Td) booster vaccination campaign for 7-year-old children in the mountainous district of Tran Yen, Yen Bai province in December 2020, observing the practice of all health workers at 4 steps (examination, counselling, injection, postinjection monitoring), each health worker observes on 10 random children and interviews all health workers at these steps to describe the current status of vaccination practice safety and knowledge of medical staff about symptoms suggesting and handing grade II and III anaphylaxis. As a result, the number of health workers who performed examination, counselling and vaccinations that could fully implement the contents at each step, for most of the observed children (≥ 8/10 children were observed) was still low, respectively 7/16, 5/16 and 3/16 health workers. The percentage of health workers with adequate knowledge about symptoms suggesting anaphylaxis, handling grade II, and III anaphylaxis, and intramuscular adrenaline regimen was 8%, 0% and 36%. It is necessary to train and assess the knowledge of health workers on safe vaccination, detection and handling of anaphylaxis before each vaccination campaign, and monitor the practice of health workers during the vaccination session to limit the following reactions, that maybe occur due to errors in the practice of health workers.
