Factors affecting goal achievement in patients receiving anticoagulation by vitamin K antagonist therapy at Hai Phong International General Hospital in 2020

Abstract

Vitamin K antagonists (VKAs) still play an important role in anticoagulation therapy, requiring international normalized ratio (INR) monitoring. The quality of oral anticoagulants, the clinical benefits, and the risks associated with VKA use are determined by achieving and maintaining an INR. Therefore, we carried out this study with the objective of this study to investigate the factors affecting the goal achievement in patients receiving anticoagulation by vitamin K antagonist therapy at Hai Phong International General Hospital. Through the analysis of 1304 electronic medical records of patients assigned to use anti-vitamin K therapy in 2020, we found that the rate of achieving the INR treatment goal in the study group of patients was 75.2%. 89.1% of patients had an INR between 2 - 3 with a mean value of 2.4 ± 0.3. Our multivariable logistic regression analysis showed that the diagnosis of mitral and mechanical valve disease and the goal of treatment influenced goal achievement. Clinical studies with more rigorous design need to be conducted to evaluate the effectiveness and safety of VKAs in the Vietnamese population in order to improve the effectiveness of treatment.
